Further, I release all parties and persons from any and all liability for any damages that may result from furnishing such information by Premier Staffing or any of its agents, employees, or representatives. I understand that any misrepresentation, falsification, or material omission of any information on this application may result in my failure to receive an offer or, if I am hired, my dismissal from employment.
In consideration of my employment, I agree to conform to the rules and standards of Premier Staffing. I further agree that my employment and compensation can be terminated at will, with or without cause, and with or without notice, at any time, either at my option or at the option of Premier Staffing. I understand that no employee or representative of Premier Staffing, other than its president, has the authority to enter into any agreement for employment relationship unless the president and I both sign a written agreement that clearly and expressly specifies the intent to do so. I agree that this employment relationship is final and fully binding, and that there are no oral or collateral agreements regarding this issue.
I also understand that all offers of emploment are conditioned on the provision of satisfactory proof of an applicant's identity and legal authority to work in the United States, as well as the satisfactory completion of a post-offer medical examination.

The Company adheres to a policy of at-will employment which recognizes that each employee and the Company each retain the right to terminate the employment relationship and that the Company retains the right to modify an employees position or compensation at any time, with or without cause or notice. No one other than the President has the authority to make any binding promise or enter into any agreement inconsistent with the Companys at-will policy and any such agreement must be in writing and signed by both parties to be effective.
As a condition of employment, all individuals offered employment are required to submit to a pre-employment drug test.
I hereby give my permission to Premier Staffing to verify the validity of my work status in the United States.
This authorization enables Human Resources to verify my social security number, my alien registration number and my employment authorization documents with the appropriate departments, such as Social Security Administration and Immigration and Naturalization.
I also realize that once I become a Premier employee, I may be asked or required to submit to testing procedures designed to detect the presence of drugs and/or alcohol if I am acting in a manner that leads to suspicion that either possess, control, or I am under the influence of a drug and/or alcohol or I have been involved in the use, possession and/or the sale of drugs or alcohal in the company controlled area, on company owned property, or while on duty.
A request for testing does not necessarily mean or imply that an employee is under the influence of any improper substance or has violated Premier's Policy. However, Such testing may be requested or required when Premier Staffing determines that it is appropriate to promote the interests of this policy. Any employee who does not consent to and cooperate fully with any search and/or medical testing procedure is subject to discipline up to and possibly including immediate termination.
I understand Premier Staffing is an At-will employer, of which the employer/employee relationship may be terminated at any time by either party.
